Your first eligible order of 250 dollars or more activates the program automatically. Credit starts accruing on that order.
Apply earned credit to future orders, up to 50 percent of an invoice, or convert it to a US Dental Depot gift card.
Your rolling 12 month average monthly spend determines the tier. The monthly minimum is what holds it. Premium is a savings tier and does not earn reward credit.
Two consecutive months without a purchase returns the account to Regular status. Credit already earned is retained.

A statement credit requires at least 100 dollars in earned credit. Once redeemed, the credit generally appears within 48 hours and the redemption is final.
Credit is returned to your rewards account provided the product comes back unopened and in a condition that allows it to be sold as new.
