What it is: A commercial service contract that covers eligible dental equipment for breakdowns after the original manufacturer warranty expires—with day‑one protection for power surge (and ADH on select devices). 

Coverage Options Available
The Buyer may choose one of the following coverage plans:
1 Year of coverage
2 Years of coverage
3 Years of coverage

Important Conditions:
Coverage must be selected as a single option.
It is not permitted to purchase multiple shorter plans and add them consecutively (for example, you cannot buy 2 years now and later add 2 more years to make 4 years).
The Buyer must decide the total coverage term at the time of purchase.

Who it’s for: Practices & labs that want predictable, contract-backed coverage on mission‑critical equipment—without guesswork or downtime surprises.

Key Benefits

Starts when the OEM stops: Parts & labor coverage begins after the manufacturer’s warranty ends and continues for 1, 2 or 3 more years 
Day‑one protection where it matters: Power surge is covered from your contract invoice date (UL‑listed surge protector required declared at the moment of purchase). ADH (Accidental Damage from Handling) is included for select categories when purchased.
Repair or replace—our call: If a covered breakdown occurs, we’ll repair or replace with an item of equal or similar features (new, rebuilt, or refurbished). Accessories not originally included aren’t part of a replacement.
Flat deductible: $100 for equipment under $2,000 & $250 for devices of $2,001 and up, per approved claim.
Clear Eligibility: Equipment must have at least a 12‑month manufacturer warranty.
U.S. coverage: Available in the 50 states (including AK & HI). Outlying U.S. territories (e.g., Puerto Rico) are excluded.

What’s Covered

Covered events: Mechanical/electrical breakdowns in normal use; power surge (with proper UL surge protection purchased); ADH for specific categories when purchased.

Parts & labor: Covered after the OEM warranty ends, for the term shown on your invoice.

Eligible equipment categories (per your invoice/Schedule of Covered Equipment):
3D printers, air compressors, autoclaves/sterilizers, delivery systems, wall‑mounted intraoral X‑rays, milling units, ovens/furnaces, patient chairs, lights, stools, ultrasonic cleaners, dry & wet vacuum systems. ADH + power surge + breakdown apply when purchased for: camera systems, digital sensors, intraoral scanners, handheld X‑rays.

Category‑specific notes (examples):
Autoclaves: Plastic patient‑facing front cover is not covered.
Delivery systems: Air/water syringes or tips are excluded. Add‑ons like intraoral cameras, handpieces, scalers require separate contracts.

What’s Not Covered


Normal wear/tear; consumables and non‑operating parts (e.g., bulbs, filters, O‑rings, water bottles, external tubing); software/data recovery; cosmetic damage that doesn’t affect function; laptops/computers/monitors (even if bundled); unauthorized repairs; environmental events (e.g., flood, fire); loss/theft; incidental or consequential losses (e.g., downtime, lost income); equipment with altered/removed serial numbers; failures due to skipped maintenance or out‑of‑spec installation/operation. SHIPPING COSTS & TRAVELING EXPENSES
Shipping costs both ways are not covered by the deductible.
Large equipment closer to 1.5 hs drive from Boca Raton Florida.Travel and visit are covered by the decutible
Large equipment further then 1.5hs drive: Standard travel charges per hour and or airfares / Hotel are not covered by the decutible.
Shipping both ways of loaners are not vovered.  

How It Works

Buy for eligible equipment (OEM warranty ≥ 12 months). Your contract term and covered items appear on the invoice. 
Maintain per OEM guidelines (you may be asked for proof at claim time). 
If something breaks: Log in at usdentaldepot.com and open the SERVICE CONTRACT TICKET CLAIM form. Wait for authorization—unauthorized repairs aren’t covered. 
Pay the deductible: $100-$250 per approved claim before service begins. 
Repair or replacement: We decide whether to repair or replace. Replacements may be new/rebuilt/refurbished and may differ in model/size/color; accessories not originally included aren’t part of a replacement; replaced items become our property; the term does not extend.

Frequently Asked Questions

1) When does my coverage start?
Power surge (and ADH where applicable) begins on your contract invoice date. Parts & labor coverage starts after the manufacturer’s warranty ends, for the remaining term on your invoice. 

2) Does this overlap with my OEM warranty?
No. Anything covered by the manufacturer during its valid warranty is their responsibility; we don’t duplicate benefits. 

3) What’s the deductible?
$100-$250 per approved claim, collected before service begins. 

4) How do I file a claim?
Log in at usdentaldepot.com, open the SERVICE CONTRACT TICKET CLAIM form, and follow the prompts. Wait for authorization before any service. 

5) What equipment is eligible?
Items with at least a 12‑month OEM warranty, listed on your Schedule of Covered Equipment/invoice. 

6) Which categories can include ADH?
ADH is not covered 

7) Is power surge covered?
Yes, from day one—provided the unit is connected to a UL‑listed surge protector. ( when o

8) Are computers covered?
No. Laptops, computers, and monitors (even if bundled) are excluded. 

9) Are consumables or cosmetic issues covered?
No. Consumables/non‑operating parts (e.g., bulbs, filters, O‑rings) and cosmetic damage that doesn’t impact function are excluded. 

10) What about added accessories on my delivery system?
Air/water syringes or tips are excluded. Add‑ons like intraoral cameras, handpieces, scalers need their own contracts. 
11) Any autoclave specifics?
Yes—no coverage for the plastic patient‑facing front cover. 

12) Will you cover data/software or training?
No. Software/data restoration, data recovery, design/training, and hardware/software upgrades are excluded.

13) Do you cover expedited shipping?
No. Expedited or overnight shipping is excluded. (Only authorized costs per an approved claim are covered; unauthorized or non‑covered costs are your responsibility.) 

14) Can I use my own technician?
Only technicians authorized/approved by the Administrator. Unauthorized repairs aren’t covered. 

15) What happens if my unit is replaced?

The replacement may be new/rebuilt/refurbished and might differ in model/size/color. Accessories not originally included aren’t part of the replacement. Replaced items become our property. Your term continues but does not extend. 

16) Is the plan renewable or transferable?
No and no. It’s not renewable and cannot be transferred to another owner or to different equipment. 

17) Where is this available?
The 50 U.S. states . Outlying U.S. territories—including Puerto Rico— & AK & HI , are excluded. 

18) Do you cover losses from downtime or lost production?
No. Incidental/consequential damages (e.g., downtime, lost income) aren’t covered. 

19) What if I can’t show maintenance records or the serial number is missing?
Claims may be denied for skipped OEM maintenance or removed/altered serial numbers. 

20) Is this insurance?
No. It’s a service contract with defined terms, exclusions, and limits.
